E-SIGN ACT DISCLOSURE AND CONSENT

Please read this E-Sign Act Disclosure and Consent (“Consent”) carefully and keep a copy for your records. To provide you with the most efficient service, Faith Based Debt Assistance and its network of debt relief partners (“we,” “us,” or “our”) need your consent to provide records to you electronically.

1. Scope of Communications to Be Provided in Electronic Form

By providing your consent, you agree that we may provide any and all “Communications” to you in electronic format, and that we may discontinue sending paper Communications to you. Communications include, but are not limited to:

  • Initial debt assessments and financial summaries.
  • Disclosures, notices, and terms related to your inquiry.
  • Privacy policies and updates.
  • Responses to your customer service inquiries.

2. Method of Providing Communications

All Communications that we provide to you in electronic form will be provided either:

  • Via e-mail at the email address you provided.
  • By access to a secure website that we will designate in an e-mail notice.
  • Via SMS/text messaging if you have provided a mobile number.

3. Hardware and Software Requirements

To access, view, and retain electronic Communications, you must have:

  • A personal computer or mobile device with internet access.
  • An up-to-date web browser (e.g., Chrome, Safari, Firefox, or Edge).
  • A valid email account.
  • Software that enables you to view and display PDF files (such as Adobe Acrobat Reader).
  • Sufficient storage space to save Communications or a printer to print them.

4. How to Withdraw Consent

You may withdraw your consent to receive Communications electronically at any time by contacting us at [Insert Email Address]. However, because our “Faith-Based” referral process is designed to work primarily online, withdrawing your consent may result in the termination of our ability to match you with debt relief providers.

5. Requesting Paper Copies

We will not send you a paper copy of any Communication unless you request it or we otherwise deem it appropriate to do so. You can obtain a paper copy of an electronic Communication by printing it yourself or by requesting that we mail you 1a copy. To request a paper copy, contact us at [Insert Phone Number]. We may charge a small fee for the delivery of paper copies.

6. Updating Your Contact Information

It is your responsibility to provide us with a true, accurate, and complete email address and other contact information. You must promptly notify us of any changes to your email address by contacting our support team.

7. Federal Law

You acknowledge and agree that your consent to electronic Communications is being provided in connection with a transaction affecting interstate commerce that is subject to the federal Electronic Signatures in Global and National Commerce Act (“E-SIGN Act”), and that you and we both intend that the Act apply to the fullest extent possible to validate our ability to conduct business with you by electronic means.

8. Termination/Changes

We reserve the right, in our sole discretion, to discontinue the provision of your electronic Communications, or to terminate or change the terms and conditions on which we provide electronic Communications. We will provide you with notice of any such termination or change as required by law.
